Important Patient Notice! We are changing our clinical computer system from  16/04/2026 to 22/04/2026. This may temporarily affect your practice experience.

Important Notice – Clinical System Downtime

Clare Road Medical Centre will be switching to a new clinical system from the 16th April to 22nd April 2026. This change has been mandated by the health board and has affected all GP practices across Wales.

From 12 noon on 16th April until 22nd April 2026, we will not have access to computerized patient medical records.

During this time:

  • We will be operating an emergency on-the-day service only
  • Routine appointments will not be available
  • We cannot access test results or medical history

Repeat prescriptions

If your medication is due during 21st April to 28th April, please submit your request in the week commencing 6th April 2026.

Eligible patients will receive 2 monthly prescriptions to help cover this period.

NHS App / Online access

Unavailable from 21st April.

If you become unwell during this time:

  • Contact the surgery in the usual way for urgent issues
  • Speak to to your local pharmacist
  • Call NHS 111 for advice.
